Kukuwa

My name is Kukuwa, I am a PCN Social Prescribing Link Worker at Lucas Avenue Practice.

As a Social Prescriber, I work closely with the GPs to help patients with non-medical and social needs.  I build relationships with my patients, listen to them carefully to find out what is important to them and to find out what motivates them.

Building this close relationship with my patients means that we can identify and overcome any issues or difficulties they may be having by connecting them with other groups and organisations in the local community. Through working together with my patients we can help to improve their overall health and wellbeing.

The areas I can help patients with include

• Mental health: emotional support, depression, loneliness, social isolation, suicidal thoughts/self-harm, low self-esteem, bereavement, grief, stress and anxiety.
• Healthy Living: obesity, eating disorder, weight loss/gain, outdoor/indoor physical activities.
• Financial advice and support: employment, benefits, eviction, homelessness, housing, cost of living, debt and basic needs.

Our hope in Lucas Avenue Practice is for our patients to have good health and a better quality of life.

Nushrath Nazar

My name is Nushrath Nazar and I’m a PCN Social Prescribing Link Worker at Carpenters Practice.

I believe that good mental health leads to good physical health.  I use a holistic approach to help and support patients by working closely with them to identify and overcome social issues by connecting them to sources of support that are available to them in the community.

Signposting and referring patients to suitable local organisations can help patients overcome barriers and allow them to access the skills they have within themselves, which in turn would leave a positive impact on their health and well-being.
